If you are a horse owner please complete our owner referral form or request that your veterinary surgeon completes the form below.

All arrangements for any referral can be entirely managed by our team here at EDC.

Once you have submitted your referral request, our reception team may contact you initially to confirm details. Chris or Nicole will then review the case and contact you through your preferred method to discuss the referral and next steps. We will keep you updated throughout the referral process, and a full written report will be sent by email following assessment or treatment. Please ensure that the email address provided is correct for the report.
